Do GLP-1s Affect Birth Control?

By OffGrid Dose Editorial Team4 min read

Tirzepatide products such as Mounjaro and Zepbound can reduce exposure to oral contraceptives after starting treatment and after each dose increase, so their labels advise switching to a non-oral contraceptive or adding a barrier method for 4 weeks. Semaglutide products such as Ozempic and Wegovy do not carry the same oral-contraceptive warning in their labels.

This is one of the GLP-1 questions where the exact medication matters. "I take a GLP-1" is not enough information. Tirzepatide and semaglutide have different labels, and birth control is not a place to average them together.

What do Mounjaro and Zepbound labels say?

The Mounjaro prescribing information and Zepbound prescribing information state that tirzepatide delays gastric emptying and may reduce the efficacy of oral hormonal contraceptives. The labels advise patients using oral hormonal contraceptives to switch to a non-oral method or add a barrier method for 4 weeks after starting and for 4 weeks after each dose escalation.

That warning is specific and practical. It does not mean tirzepatide cancels every form of birth control. It means oral hormonal contraceptive exposure can be affected, especially around starts and dose increases.

What about Ozempic and Wegovy?

Ozempic and Wegovy contain semaglutide. Their labels discuss delayed gastric emptying and possible effects on absorption of oral medications generally, but they do not include the same 4-week oral-contraceptive warning that appears in tirzepatide labels.

Still, vomiting or severe diarrhea can affect oral medication reliability for reasons unrelated to the GLP-1 label. If you vomit after taking a pill or have ongoing diarrhea, follow your contraceptive's instructions and contact your clinician or pharmacist.

Quick comparison

MounjaroTirzepatideUse non-oral contraception or add barrier method for 4 weeks after start and each dose increase if using oral hormonal contraception
ZepboundTirzepatideSame tirzepatide oral-contraceptive warning
OzempicSemaglutideNo matching 4-week oral-contraceptive warning in label
WegovySemaglutideNo matching 4-week oral-contraceptive warning in label

Confirm with your own medication label and clinician because product instructions can change.

What if you are switching medications?

Switching from semaglutide to tirzepatide can change the birth-control conversation. If you move from Ozempic or Wegovy to Mounjaro or Zepbound and use oral hormonal contraception, ask about backup contraception before the first tirzepatide dose.

Dose escalation matters too. Tirzepatide is usually increased gradually. The label warning applies after starting and after each dose increase, not only the first shot.

Pregnancy planning and GLP-1s

If you are pregnant, trying to become pregnant, or could become pregnant, talk to your prescriber before using a GLP-1. Product labels include pregnancy-related warnings and advise clinician-guided decisions.

Weight loss medications are not something to casually continue during pregnancy planning. Your clinician can discuss stopping timelines, alternative treatment, diabetes management if relevant, and contraception choices.

Track dose increases so backup timing is clear

If you use tirzepatide and oral contraception, dose escalation dates matter. A note that says "increased to 7.5 mg on June 4" is more useful than trying to reconstruct it from memory later.

Frequently Asked Questions

Does Mounjaro make birth control pills less effective?

The Mounjaro label says tirzepatide may reduce the efficacy of oral hormonal contraceptives and advises non-oral contraception or a barrier method for 4 weeks after starting and after each dose increase.

Does Zepbound affect birth control pills?

Yes. Zepbound has the same tirzepatide oral-contraceptive warning: use a non-oral method or add a barrier method for 4 weeks after starting and after each dose escalation.

Does Ozempic affect birth control pills?

Ozempic does not have the same 4-week oral-contraceptive warning as tirzepatide labels. But vomiting or severe diarrhea can affect oral medication reliability, so ask your pharmacist or clinician if that happens.

What birth control works best with GLP-1s?

Ask your clinician. Non-oral methods avoid the tirzepatide oral-absorption issue, but the best option depends on your health history, preferences, pregnancy plans, and risks.
